Rarity is the color of an item's name. The rarer it is, the more mods it has:

1Common
2Uncommon
3Rare
4Epic
5Legendary

Rarity decides how many mods an item has, not which ones.

1/500of the gear that drops is legendary
×3rarity chance in dungeons

Gear you craft also rolls a rarity. Your profession level, your Charisma and the dragon hammer raise it.
